Individuals, age sixteen years and older, are matched to a family caregiver who provides room, board, and daily personal care. Both the caregiver and the participant are assessed to determine the type of care that is to be provided. Personal interests are determined and become part of the care plan. The program social workers and nurses oversee the placements to ensure their success. The placement is afforded either with total private payment or partly with MASSHEALTH reimbursement.
