A shared living environment that combines housing with supportive services such as case management, homemaking and personal care assistance. Residents have private bedrooms but share living, dining and kitchen areas. ELIGIBLE: Individuals who need social and health services to maintain independent lifestyles but do not require 24 hour supervision. Individuals must also meet income guidelines.
